About the Author:

Kristofer J. Hagglund, PhD, ABPP, is the Associate Dean of Health Policy and Academic Affairs and Professor of Health Psychology and of Public Affairs at the University of Missouri-Columbia. He is also the codirector of the Center for Health Policy, a nonpartisan research and policy analysis organization committed to improving health and health care. He was a 2000-2001 Robert Wood Johnson Health Policy Fellow in the Office of Senator Tom Harkin (D-IA) and was President of the Division of Rehabilitation Psychology of the American Psychological Association in 2003-2004 and was awarded the James Garrett Early Career Achievement Award by the Division in 1997. Dr. Hagglund has an active research program and has published more than 50 manuscripts in the areas of chronic illnesses and disability, health care delivery systems, and public policy. He is the co-principal investigator of the Missouri Model Spinal Cord Injury System and a co-principal investigator of the Missouri Arthritis Rehabilitation Research and Training Center, both funded by the National Institute on Disability and Rehabilitation Research. He is also the co-principal investigator of a project addressing racial and ethnic health disparities that is funded by the Missouri Foundation for Health. Dr. Hagglund obtained his doctoral degree in clinical (medical) psychology from the University of Alabama at Birmingham. He is a diplomate of the American Board of Professional Psychology and a Fellow of the American Psychological Association.

Allen W. Heinemann, PhD, ABPP, completed his doctoral degree in clinical psychology at the University of Kansas with a specialty focus in rehabilitation. He completed an internship at Baylor College of Medicine and was an assistant professor in the Department of Psychology at Illinois Institute of Technology. Since 1985, he has worked at the Rehabilitation Institute of Chicago, where he directs the Center for Rehabilitation Outcomes Research, a rehabilitation-focused health services research unit. He is also associate director of research at RIC and professor in the Department of Physical Medicine and Rehabilitation at the Feinberg School of Medicine, Northwestern University. He serves on the Coordinating Committee for Northwestern's Institute for Healthcare Studies. He is the author of more than 110 articles in peer-reviewed publications and is the editor of Substance Abuse and Physical Disability. He is the recipient of funding by the National Institute on Disability and Rehabilitation Research, the National Institute on Alcohol Abuse and Alcoholism, Centers for Disease Control, and the Social Security Administration. During 2004 and 2005, he served as president of the American Congress of Physical Medicine and Rehabilitation and the Rehabilitation Psychology division of the American Psychological Association. He is the recipient of the Division 22 (Rehabilitation Psychology--of the American Psychological Association) Roger Barker Distinguished Career Award.
